Embarrassment is a genuine health risk. Since these are exactly the symptoms where early assessment matters, it's worth pulling back the curtain on what actually happens, because the reality is far more ordinary, quicker, and more dignified than the anticipation.

What the appointment actually involves

It starts, like any consultation, with a conversation — your symptoms, their history, your general health — fully clothed, across a desk. The examination, when it comes, is brief and staged. You'll have privacy to undress from the waist down and a sheet to cover yourself; a chaperone is offered and available. You lie on your side with your knees drawn up. The doctor inspects the outside, then usually performs a gentle internal examination with a lubricated gloved finger, and often a proctoscope — a short, smooth, lubricated instrument that allows the anal canal and lower rectum to be seen. It's quick, typically a couple of minutes, and while undignified-feeling, it is not usually painful. For the person doing it, this is entirely routine — many times a day, every working day.

Why it's worth overcoming the dread

Two things make the fear worth pushing through. First, most bottom symptoms have benign, treatable causes — piles, fissures, skin conditions — and many can be diagnosed and even treated at that first visit; piles can often be banded on the spot. Second, the symptoms people are most embarrassed about — bleeding especially — are exactly the ones that shouldn't be sat on, because on the small chance something needs finding early, early is when it's most treatable.
